Liability Waiver

I understand this is an important legal document relating to my participation in today’s FCA DC event held at the Fields at RFK and by signing this document. I am waiving all legal rights I may have against the Fellowship of Christian Athletes and the Fields at RFK.

Physical exercise can be strenuous and subject to risk of serious injury. I hereby acknowledge that I can participate at my own level of comfort and can stop at anytime. FCA DC urges that all participants obtain medical consent from a medical doctor before participating in today’s event. You agree that by participating, that you do so entirely at your own risk. You agree that you are voluntarily participating in these activities and in the use of these facilities, equipment, and premises and assume all risks of injury, illness, or death. 

Social Media Release

I hereby grant FCA permission to use my likeness in photograph(s) or video(s) in all its publications, its website, and in all other social media including Instagram, whether now known or hereafter existing, controlled by the FCA DC staff, in perpetuity. I will make no monetary or other claim against FCA for the use of the photograph(s) and/or video(s).

You acknowledge that you have carefully read this waiver and release and fully understand that it is a release of liability. You expressly agree to release and discharge FCA DC for any and all claims or causes of action and you agree to voluntarily give up or waive any rights that you may otherwise have to bring a legal action against FCA DC for personal injury and property damage. 

By signing this release, I acknowledge that I understand its content and that this release cannot be modified orally.
